Please use these Disability Services student forms to apply for different types of accommodations at Luther College.
If you have any questions about which form to use for your accommodation request, please contact Disability Services staff.
Request an Accommodation
Use this form to request academic, dining, or housing accommodations. In addition to the form, there is a unique process for each type of accommodation request.

Release of Information
Use this form to request a release of information from a medical provider to be sent to Disability Services. This form should be completed and sent to Disability Services at disabilityservices@luther.edu or faxed to 563-387-1411.

Medical Documentation
Use this form to provide documentation from an appropriately-licensed medical provider. Students are required to provide documentation which supports their accommodation request in order to have their request reviewed. The form is to be completed by the student and the medical provider. Completed forms should be sent to disabilityservices@luther.edu or faxed to 563-387-1411.

Dining Accommodations Documentation
Use this form to provide documentation for a dining accommodation request. The form is to be completed by the student and the provider. Completed forms should be sent to disabilityservices@luther.edu or faxed to 563-387-1411.

FERPA Waiver
Students who wish to give parents or guardians access to information in the student’s accommodation files, or to talk with Disability Services about their accommodations must complete a FERPA Waiver Form. This form can be found at the Registrar’s office.

Grievance and Appeal
Use this form if you would like to file a grievance or an appeal of your accommodations or accommodation request.

Request Audible Textbooks
Use this form to request audible textbooks for a class. Students must be approved through Disability Services with an accommodation to request audible textbooks. Once textbooks are requested, disability staff members will complete the request in a timely manner.

Request a Notetaker
Use this form to request a Notetaker for a course. Students must be approved through Disability Services with an accommodation to use notetakers. Notetakers will be assigned in a timely manner.
